Inspection methodology involves a team of inspectors, including clinical and social care specialists, reviewing documentary evidence submitted by the provider, interviewing staff, patients, and relatives, observing care being delivered, and assessing the provider's systems and processes against the applicable regulations and fundamental standards. Enforcement powers available to the CQC include issuing warning notices that require providers to improve in specified areas within a set timeframe, imposing conditions on registration, restricting or suspending a service, cancelling registration altogether, and in the most serious cases referring matters for prosecution or to professional regulators. Registration as a condition of providing regulated activities means that services cannot operate legally without CQC registration, giving the regulator a fundamental lever over the sector. NHS trusts and foundation trusts are subject to a distinct inspection regime that assesses services at the core service level and then produces an aggregated rating for the trust as a whole.
